- All guests must be referred by a hospital personnel where the patient is actively receiving treatment. This may include a physician, nurse, social worker, or chaplain. If the patient is not currently at the hospital but will be arriving for a scheduled treatment in the future, guests must contact their physician’s office who is scheduling the treatment to request a referral form. Hospital staff and physician offices must complete a Hospitality House of Charlotte Referral Form and return it our office by fax at 704.376.0059 or by email at email@example.com.
- Once a referral is received, guests will be placed on a waiting list for the expected arrival date. It’s important to note that the receipt of a referral does not guarantee a room due to the high number of referrals we receive.
- On the day of your expected arrival, we will contact you after 12 p.m. and let you know if a room has become available. Check-out for guests is at noon so we will contact you after this time since we know how many rooms have become available.
